Childcare Request Form

Unity Church offers childcare for church-wide events, free of charge. Nursery care is consistently provided for every Sunday service and Wellspring Wednesday program -- no reservation is needed.

Outside of the above times, reservations for childcare are required. Please request childcare one week ahead of time by filling out the following form. Katrina Nichols, Unity's Nursery Coordinator, will contact you to confirm the reservation.
